JOB SUMMARY:
Clinical Pharmacist-Oncology is responsible for reviewing and evaluating the chemotherapy prescription as per the Standard Protocol and guidelines in supportive medications. He /She will review the inpatient’s active profile, and identify and resolve drug-related problems. He /She will also do drug utilization reviews. He/ she provides clinical monitoring and provision of drug information in a manner that ensures safe, appropriate, and cost-effective utilization of pharmaceutical products.
- Teaching of trainees and students as and when required
- Actively participates in gathering accurate data for quality assurance issues
- Ensures all documentation is retained in an organized manner as per the retention policy of pharmacy
- Participation in conducting technical training and competency assessments.
- Implements all safety regulations on a daily basis for own safety and safety of others.
- Provides recommendations to management regarding the updating of the hospital formulary
- Assists Chief Pharmacist-Oncology in updating and developing new policies and procedures followed by effective implementation
- Keeps up-to-date with current techniques and developments in Oncology Pharmacy procedures and participates in trainings as required
- Preparation of daily reports regarding Clinical Round observations OR intervention documentation.
- Any other task assigned by Chief Pharmacist-Oncology
- Drug therapy problems: Number of drug therapy problems addressed by a pharmacist per admission.
- Attendance and punctuality
- Performs ward rounds to assess the appropriateness and effectiveness of medications prescribed as per the clinical requirements of patients
- Review the physician order for an accurate therapeutic dose, proper indication, drug interaction, contraindications, allergies and duplication of same class etc. Contact the physician for rectifying the prescription order and do proper documentation
- Efficiently performs other pharmacy duties including the safe procurement, preparation, labeling, distribution, and control of all drugs.
- Ensures medicinal products are stored appropriately and securely to ensure freshness and potency.
- Advises physicians on the selection, dosage, interactions, and side effects of medications
- Provide drug and poison management information to healthcare staff and patients
- Provide education to patients and their families about medication side effects and other concerns related to treatment.
- Calculation of TPN for adult and pediatric populations
- Cost Containment strategies by doing clinical reviews such as IV to oral Switch, and dose rounding of chemotherapeutic drugs.
