The Domiciliary Care Service is a home based assessment and maintenance program for frail, older and younger disabled people and their caregivers who live at home or in the home or residence.
The focus of our program is to promote and maintain patient independence in the home and community environment by providing advice and support to them and their caregivers.
PHYSIOTHERAPY, NURSING AND LAB SERVICES:
The Physiotherapy and Nursing Domiciliary Service provides a home quality service to help patients continue to live at home or in their nursing or residential home so they can be as independent as possible.
Domiciliary Physiotherapy service provides fitness rehabilitation and self- management strategies for the patients/clients within their own homes. Our qualified staff aims to facilitate patients to improve their movements and functions through multidisciplinary rehabilitation process. Our Qualified physiotherapists, uses manual therapy, electrotherapy and other movement and exercise techniques for patients affected by injury, illness or disability. A treatment plan is formed, based on the patient centered goals.
We aim to reduce unnecessary hospital admissions by addressing the patients’ problems in their own home by assessing, treating, advising and providing aids and equipment as necessary.
Our trained healthcare professionals work in many areas including:
- Respiratory therapy
- Post op care
- Education and Health Promotion
With our special home nursing care service we also support patients with long term conditions by helping them to manage their own condition as and when it changes.
The frequency of treatment will depend on your needs. When your treatment is complete, you will be discharged from the service. However, the Physiotherapist or nursing care provider will often leave written advice for you to continue following.
In addition our lab services cater to patient’s convenience by making collections at home at any given time.
Appointments And Inquires:
Referrals are welcomed from GPs, consultants and hospital therapists. Patients are also welcome to self-refer.
