Instructions
 
  1. Life Style Executive clinic is open from Monday through Friday from 9:00 AM to 3:00 PM
  2. Please call a day before to confirm your appointment and to register yourself at the Life Style Executive Clinic.
  3. In package 'A' & 'B' , twelve hour fasting is required prior to the appointment, i.e if your is at 9:00 AM. then please do not take any food ar beverage after 9:00 p.m. the day before . However, you can drink water during fasting.
  4. Specimen containers should be collected prior to the first visit from the Hospital.
  5. Specimen of stool and urine should preferably be brought from home on the first visit to the clinic.
  6. In package 'A' & 'B' Breakfast will be served after the blood test has been completed.
  7. In package 'A' & 'B' , doctor's consultation will follow in a week after the intial visit.
  8. In package 'A' & 'B' two visits are required, the first to run all the tests and the second for the doctor's consultation.
  9. Medical report will be dispatched to your calling address within one week from the date of your doctor's consultation.
  10. If your company is sponsoring your Lifestyle Executive Clinic visit, please make sure that you bring a letter of authorization on the day of your first visit. Otherwise cash payment in full will be required.
